I am trying to open rqt\_graph on my Host machine but it gives me the following error:
QXcbconnection: Could not connect to display

I tried to ssh using -y and -x but it still gives the same issue

Do you have a display on said remote machine? Rqt\_graph does require a display to open the gui element.

Sorry, I meant on the host machine,
So in this case basically the raspberry pi would either need to be connected using a vnc or basically adding a display to the raspberry pi?
